Generic ibuprofen is Advil. The same pill, a fifth of the price.

Generic ibuprofen is Advil, generic acetaminophen is Tylenol, generic naproxen is Aleve: the FDA requires the identical active ingredient at the same dose. Yet the brand box costs up to six times more per dose. So the honest way to buy pain relief is by cost per dose, not by the name on the bottle. Here is every common OTC option ranked by real value.

Prices reviewed 31 July 2026 . Cost per standard adult dose, in USD; your dose may differ. Not medical advice, ask a pharmacist or doctor about what is right for you.

Brand pain relievers cost about 5× more than the identical generic

We costed 11 OTC pain relievers by cost per dose. Generic acetaminophen is $0.04 a dose, the exact same drug as Tylenol at $0.24; generic ibuprofen is $0.05 versus Advil; generic naproxen undercuts Aleve. All generics are FDA-bioequivalent, the same medicine, so the brand premium buys only the name on the box.

Generic and brand versions of each drug are the same active ingredient at the same dose and FDA-bioequivalent, so they work identically. How this works. Per dose is the cost of one standard adult dose. The key fact: generic and brand versions of the same drug, ibuprofen and Advil, acetaminophen and Tylenol, naproxen and Aleve, contain the identical active ingredient at the same dose and are FDA-bioequivalent, so they work exactly the same. The generic simply costs several times less, and store brands (Equate, Kirkland, up&up, Amazon Basic Care) are the same drugs for a hair more than bulk generic. Which drug to pick depends on the problem: acetaminophen is gentle on the stomach and good for general pain and fever; ibuprofen and naproxen are anti-inflammatories, better for swelling, with naproxen lasting up to 12 hours; aspirin works but is harder on the stomach and thins the blood. Never exceed the label dose, and do not double up on two products with the same ingredient. This is information, not medical advice, check with a pharmacist, especially with other medicines or stomach, kidney or heart conditions.

Common questions

Is generic ibuprofen the same as Advil?

Yes, the identical drug (ibuprofen) at the same dose, FDA-bioequivalent. A generic or store-brand tablet is a few cents versus 10 to 15 for Advil, often 50 to 75 percent less. Same for acetaminophen and Tylenol, naproxen and Aleve.

What is the cheapest pain reliever?

Bulk generic acetaminophen and ibuprofen, a few cents a dose. Store brands are the same drugs for a little more. Aspirin is cheapest per milligram but is harder on the stomach and thins the blood, so not the right first choice for everyone.

Which pain reliever should I use?

Acetaminophen (Tylenol) for general pain and fever, gentle on the stomach. Ibuprofen (Advil) and naproxen (Aleve) are anti-inflammatories for swelling and joint pain, naproxen lasts up to 12 hours. All are cheap generics. Ask a pharmacist if unsure.

How much can you save going generic?

A brand dose is about 22 to 24 cents versus 4 to 5 for the generic, roughly five times less for the same drug. Small per dose but it adds up, and there is no reason to pay the brand premium for identical medicine.
