Wacom tablets are robust, but the pen is where problems hide and a replacement pen can cost a large share of a used tablet's price, so confirm the pen is included and fully working: it should track smoothly across the whole surface, register pressure sensitivity, and its buttons should work. A tablet sold without its pen is only worth buying if you already have a compatible one.
Nibs wear but are cheap. The pen nib (tip) wears down with use and can scratch the surface once too short, but replacement nibs are inexpensive and often come in a multipack, so a worn nib is a minor point. Check the tablet surface itself for deep scratches or gouges, which do affect drawing feel and cannot be undone.
